Good Morning,

 

If you are receiving this email, we are missing your camper’s health form upload and a copy of your Insurance Card (if applicable).  Below are the specifications on how to submit these forms.  If you have any questions please do not hesitate to give me reply.

 

 

Camp physicals within 12 months of this email are acceptable.

 

What types of physicals does Camp Wildwood accept?

• A yearly physical within the last 12 months

• A school sports physical within the last 12 months

• Visit your family doctor and ask for a camp physical.

• CVS Minute Clinic offers a camp physical at your convenience.

• We accept any standard sport/camp physical form that the doctor's office or school provides, but if you need our standard form to take to the physical, you may use this one.

 

SENDING UPLOADS:

 

Send the health form and a copy of your insurance card to the Camp Wildwood Office (3 Different Ways)
